The original agreement will be retained by ESI-PGIMSR, ESIC Medical College and ESIC Hospital and ODC (EZ), JOCA and the candidate will retain one copy. It will be the responsibility of the candidate to bear the cost of stamp paper.
For teaching faculty: Pay the application fee (₹500 for others, ₹0 for SC/ST/ESIC employees/Women/Ex-Servicemen) through demand draft in favor of “ESI Fund Account No. I” payable at Kolkata.
